1. Proof Of the minor’s date of birth 
2. Passport-size photographs of the applicant minor and of parent/guardian as well. 
3. Proof Of Identity - KYC documents of parents/guardians such as Aadhar card, PAN card, Driving License, and Voter’s Identity Card. 
4. Proof Of Address - Passport, Aadhar card, Driving License etc., 
5. Aadhar card of the minor (if applicable) 
6. Specimen signature of a guardian. 
7. The minor’s specimen signature if he/she is 10 years old or above.
